Born Keldon Wilder Johnson on October 11, 1999, in Chesterfield, Virginia, he attended Park View High School as a freshman and sophomore, breaking the Virginia High School League ninth-grade scoring record. He transferred to Huntington Prep for his junior year and Oak Hill Academy for his senior year, where he was named the Virginia Gatorade Player of the Year and selected to the McDonald's All-American Game and Jordan Brand Classic.

At Kentucky, Johnson averaged 13.5 points and 5.9 rebounds as a freshman, shooting 46.1 percent from the field and 38.1 percent from three. He scored 19 points in a win over top-ranked Tennessee and recorded a career-high 17 rebounds against Auburn. He declared for the 2019 NBA draft.

The San Antonio Spurs selected Johnson with the 29th overall pick in the 2019 draft. He signed with the Spurs on July 1, 2019, and made his NBA debut on November 22, 2019. On January 17, 2023, he scored a career-high 36 points with 11 rebounds in a win over Brooklyn. On July 18, 2022, he signed a four-year, million contract extension with San Antonio. He was named the NBA Sixth Man of the Year in 2026 and helped the Spurs reach the NBA Finals that same season.
