Born on February 20, 2004, in Corona, California, McCain attended Centennial High School. As a junior, he was named the California Gatorade Player of the Year after averaging 16.9 points, 4.7 rebounds, and 4.0 assists. He appeared in three seasons of the YouTube docuseries Believe. As a senior, he repeated as California Gatorade Player of the Year, averaging 17.7 points, 7.1 rebounds, 4.0 assists, and 1.5 steals. He was selected to the McDonald's All-American Game and committed to Duke over Gonzaga, Houston, Kansas, and Louisville.

At Duke, McCain made his debut on November 6, 2023, and quickly established himself as a scoring threat. On February 17, 2024, he scored 35 points against Florida State, tying the Duke freshman single-game record previously held by Zion Williamson. In the Elite Eight of the 2024 NCAA tournament, he scored 32 points in a loss to NC State. He averaged 14.3 points and 5.0 rebounds for the season.

The Philadelphia 76ers selected McCain with the 16th overall pick in the 2024 NBA draft and signed him to a four-year contract on July 4, 2024. He made his NBA debut on October 23 and recorded a career-high 34 points and 10 assists on November 13 against Cleveland. He was named Eastern Conference Rookie of the Month for October and November, becoming the first 76ers player to receive the honor since Ben Simmons in 2018. On December 14, 2024, he suffered a meniscus tear in his left knee and missed the remainder of the season.
